Care Navigator - Part Time

Catholic Charities has an exciting opportunity to be part of an innovative new program assisting caregivers of individuals with Alzheimer’s or other forms of dementia.
The Part-Time Care Coordinator will work closely with families and caregivers providing care coordination that will link them with needed services and supports. Responsibilities include: establishing connections with families and caregivers, conducting home visits, and conducting intakes; developing a care plan; monitoring care plans; communicating regularly and ongoing case conferencing; advocating with community partners; coordinating services; and forming strong relationships with community providers.
A Bachelor’s Degree in Social Work or a related field and three years of experience in case management, care coordination, or caregiver support services or a Master’s Degree and two years of experience is required. A valid driver license and access to a vehicle during all work hours is also required.
